To use this template, simply click the "Do It" button. It will prompt you with a few simple questions and then will automatically create your plan and list of references and actions.

Customized Nutritional Plan (CNP)  -  $99.00
Whether your goal is to lose weight, reduce body fat, gain lean mass, eat healthier or just feel better, Nutrition Coach TK can help you reach your goals! They say fitness is 80% food and 20% exercise. That’s why the first key to any fitness program is the correct nutritional guidance. This package is designed to get you started quickly toward your goal.

Package includes:
Upon signing up for the Customized Nutritional Plan (CNP) package you will be sent an online Lifestyle Profile Survey and a 4-Day Food Log. Once you have completed your Lifestyle Profile and 4-Day Food Log, Coach TK will provide written recommendations on how to make simple adjustments to your current eating habits and lifestyle to help you reach your goals. The recommendations will include specific food adjustments, meal ideas, recipes and exercise recommendations to get you started. In addition, a 45 min call will be scheduled to review the recommendations and go over any questions.

8-Week Individual Nutrition Coaching  -  $375.00
This package is perfect for someone with a specific goal and timeline – whether an upcoming special occasion, vacation or event. Whether your goal is to lose weight, reduce body fat, gain lean mass, eat healthier or just feel better, this package offers a Customized Nutritional Plan and one-on-one individualized coaching. This program will give you a better understanding of nutrition and help you reach your fitness goals. It also provides ongoing coaching that holds you accountable each week so that you stay on track with ongoing adjustments to your plan.

This comprehensive program includes:
* Lifestyle Profile Survey & 4-Day Food Log Review

* Initial Kick-off Session (up to 1 hour via phone) where we will discuss your goals, current eating & exercise habits, nutrition basics and your specific nutritional plan for the 8 weeks we will spend together.

* Weekly Check-in Calls - Each week, Coach TK will review your Progress, Food Log, questions and provide feedback and adjustments via phone or email for the following week to assure you stay on track with your goals.

groupLOSS  -  $135.00
There is nothing more difficult when you are trying to lose weight than going to the office and seeing a bunch of goodies in the kitchen, or going to lunch with co-workers that are eating things that are not healthy. Or how about going out with your friends and you are watching them eat and drink whatever they want while you make healthy choices?

Well, what if you were all on the same plan and actually supported each other? Wouldn’t that be more fun (oh and productive)? What if you could all lose some weight? What if you all had more energy and felt great all of the time? Wouldn’t that be more fun?

Having people around you supporting your goal is key in any weight loss effort. groupLOSS is an 8-week online group coaching session designed for small groups of people with the same goal…to lose weight. But not just to lose weight, to lose weight in the right way (through education & slow easy changes) and for the long term.

groupLOSS is ideal for the office, clubs, social groups and friends. As a group, you will work together and support each other (with my help) to reach your group’s goal, it’s a team effort! Here is how groupLOSS works:

SIGN-UP: get a group of friends together (co-workers, gal pals, poker buddies, tiddlywink group…,whatever) and sign up
SETUP: each member will fill out a private lifestyle survey and complete a 3-day food log. I will review each person’s lifestyle survey & food log and develop a custom 8-week plan for the entire group and post it on the private groupLOSS home page.
GET STARTED: I will coordinate a short skype conference call to review the plan and go over any initial questions (for those that cannot make the call, all the notes will be posted on the groupLOSS home page and they can review at anytime).
CHECK-INS: Every week, people will check-in and provide (privately via email or groupLOSS form) their weight and food log.
UPDATES: After I have reviewed the results from the previous week, I will post total weight loss* (see NOTE below) on the groupLOSS weight loss meter and provide feedback, recommendations and tips to keep you all on track with your group goal.
FINISH: By the end of the 8 weeks, everyone in the group will have learned why traditional diets usually fail (long term) and the best approach for long-term weight loss – in addition to losing between 4-16 lbs (or more) each.

*Minimum group size is 4.

*PRICING: Pricing is based on a per person basis. Each person participating in your groupLOSS is responsible for their setup fee and monthly fee (unless otherwise arranged). Setup charge is $57 (and covers client lifestyle survey and food log review and incorporation to group plan), then $39/mo for 2 months. Total 8-week cost $135/person. Set-up fee and 1st month is required to start. For monthly billing options companies wishing to pay for all group members at one time please email a request to tk@hereswhatido.com – thank you!

*NOTE: Individual weight and weight loss and food logs will not be shared with the group (unless the group choices to do so). Total weight loss for the group will be tracked and shared with the group each week.

* Weight loss of 4-16 lbs is on average, individual weight loss will vary.
